Healthcare Professional Referral Form

This referral pathway is intended for GPs, physiotherapists, and other healthcare professionals wishing to refer patients into Horizon’s Exercise Referral Scheme.

Please complete the referral form, ensuring all relevant clinical information is included. Once completed, Horizon will arrange patient onboarding.

The programme is delivered at our Havant and Waterlooville centres in partnership with Havant Borough Council, the NHS, and Macmillan Cancer Support.
