Welcome to our Health Referral Scheme. At Horizon Leisure, we’re here to help you increase your physical activity levels and improve your overall health. Join us for our 12-week structured course of physical activity, designed to help you move a little more every day in a fun and supportive environment.

How to join

The scheme is in partnership with Havant Borough Council, the NHS and Macmillan Cancer Support. Participants are referred to the exercise referral scheme through a health professional or GP where physical activity can help to:

Ask your GP or healthcare professional to refer you, then we’ll contact you to book your first session and talk through what to expect.

Health referral form

Download the referral form to take to your GP or healthcare professional. Once it’s completed, they’ll send it to Horizon so we can get you booked in.

Health experts run the scheme at our Havant and Waterlooville centres in partnership with Havant Borough Council, The NHS and Macmillan Cancer Support.

Eligibility and Referrals

Horizon’s Health Referral Scheme is for adults aged 16+, referred by a GP or healthcare professional, where safe physical activity could support their health. To join, you must meet the scheme criteria and have no absolute contra indications to exercise.

Medical Conditions

The scheme may support people living with a wide range of conditions including hypertension, diabetes, high cholesterol, mental ill health, asthma, arthritis, osteoporosis, cancer recovery, cardiac rehabilitation, chronic fatigue, lower back pain, Parkinson’s, and joint rehabilitation, as well as those with a BMI of 25+ alongside other risk factors.

Absolute Contra Indications

Some conditions mean exercise may be unsafe without further medical support, such as unstable heart conditions, very high resting blood pressure, uncontrolled tachycardia, severe or poorly controlled asthma or COPD, uncontrolled epilepsy or diabetes, uncontrolled arrhythmia, aneurysm, or febrile illness.

Referring a Patient (for professionals)

Healthcare professionals should explain the scheme, confirm the preferred Horizon centre, and complete the digital referral form. Once received, Horizon will contact the patient to book their first appointment.

What Can I Expect From The Health Referral Scheme?

With our Health Referral Scheme, you’ll have a dedicated exercise referral instructor by your side, guiding and supporting you throughout your journey. They’ll create a personalised activity plan tailored to your health status, preferences and lifestyle, while also providing access to discounted facilities. You can count on regular check-ins with your instructor to review and adjust your plans as needed too.

On completion of the 12 week programme, there is the opportunity for continued discounted membership at Horizon Leisure.

Your instructor will check in with you throughout the programme, reviewing your progress and adapting your plan when needed.

ESCAPE-Pain Course

ESCAPE-Pain is a group rehabilitation programme for people with Osteoarthritis of the knees and/or hips, that integrates educational self-management and coping strategies with an exercise regimen individualised for each participant. It helps people to understand their condition, and teaches them a progressive exercise programme so they can learn how to cope with their pain better.

ESCAPE-Pain is for anyone aged 45-years or older with chronic joint pain caused by osteoarthritis of the knees or hips, for at least the last 3 months.

The course is twice a week for 6 weeks, and the attendees must attend both days. We alternate courses between Havant and Waterlooville Leisure Centres.

It is £50 per course, which will be paid at the start of the course.

Yes. You will need to be referred by a GP or another healthcare professional to join the scheme.

No. The programme is designed for all abilities, including people who are brand new to exercise or returning after time away.

Your plan will be tailored to you, but may include gym based exercise, strength and mobility work, low impact cardio, and other suitable activities at Horizon.

At the end of the programme, you may have the opportunity to continue with a discounted membership so you can keep building on your progress.
